I've downloaded r250 and will try it. I did make the change for last night's raid to extend the time on the talent checks as you suggested a page or two back and didn't get an error.
Good. That confirms it's something to do with talent re-scanning.
I looked at the error message referenced above, and both that person and I use TipTop. There are several other mods in common, of course, but TipTop also does some talent inspection. Perhaps there is some conflict?
Yes quite possibly. Good spot. There has to be something else to explain it as I never ever get the problem and raid and wipe and change spec and teleport 4 nights a week. Nor does anyone else in the guild. I will grab a copy of that addon. But the latest version should give some more incite to the error.

Just to be clear. I don't expect r250 to fix anything. But when it does go wrong it should give more information on the problem. Which you can then report back. Then maybe I can fix it or have more ideas :)
Also be sure to get the latest alpha before the raid in case I have some more ideas before then.
Finally! Yes the problem is the TipTop addon! I can now reproduce the problem. You do not need to keep testing it for me. I will find out what is wrong and let you know when I know it is fixed.

No. Check the talents window that RBS does not think one of the druids is hybrid or feral. For that druid check you can inspect his talent points manually.

RBS tries to automatically spot who the tanks are. It does this in two steps. First it looks at the tank list. (Every raid uses a tank list right?) It will ignore players who are not tanks (eg Priests) that are only being used for following DPS or other scenarios.

Was the tank prot spec? For a warrior to be considered a tank they need to be on the list and prot spec. Please double check what spec RBS says they are. Also enable the RBS tank list button to see who RBS considers to be tank. Let me know what you find. If needed I could add an option to only use the tank list and ignore spec.

im not sure wether the whole raid gets the combatlogevent for spell_cast &spell_create, but if they do, you could get rid of the translation and listen to that
