I have a feeling this could very quickly and very easily become the one mod I can't live without. On a very brief first pass, this is exactly what I've been hoping to find and seems to function exactly how one would expect it to function. Thank you thank you thank you.

For those that dont know you can make a cirle of buttons. Think minimap icons. And when you hit a key binding the sprocket pops up. Then you can move the cursor to a portion of the pie so to speak and it will select that action and when you release the keybinding it will use that action. Words really cant describe it though it is very nice to look at and easier to understand seeing it.
Pretty sure it's still a wip. There's definately things it still needs. Scaling for one but I honestly didn't look to hard at it yet so there could be scaling. One thing id like to see is a central button there's a lot of wasted space in there. I would love to replace ghostbar mod with sprocket. A option to not use keyup would be nice to. For example I use C to bring up the ghostbar mod and sometimes I dont mean to use it and accidently hit the button. Also drag and drop onto the sprokets would be nice but not really needed.
does it discern between right and left click for applying to main and offhand?
No, it just put's the "apply this poison" as an action on your mouse.
Fyi, this is quite possible I believe. You could either setup 2 hotkey's, one per weapon, or a single mouse trigger that opened the same menu on LeftButton and RightButton but with different post actions on each.
In either case, you can set the post action on your triggers to type Script, and use the appropriate script command to apply the poison to your weapon of choice. Specifically, it looks like you'd do the following for the Post Actions:
"/script PickupInventoryItem( 16 )" for main hand.
"/script PickupInventoryItem( 17 )" for off hand.
