I've always wanted a time-degrading ignore list. For most cases, "report spam" will do... but if I get a really shitty PUG idiot I'd like to ignore him, and have that ignore automatically go away 3 or 6 months down the line. I doubt I'd ever hit the limit, but if I did "drop the oldest" would be nice.
Throwing my name in the hat too. Anal/baby spam and bad puggers are overwhelming these days. I would love the following:
1) An unlimited length ignore list. If possible, I assume it's not. Maybe it would have to be clientside only?
2) Remove the oldest ignore when a new one is added at the limit
3) Automatically remove ignores older than X days (as requested earlier in the thread)
4) Method to tag an ignore as permanent so it's never automatically removed or aged off. Permanent ignores would always go on the serverside list. I suppose this could be seen as a binary severity description as requested in the OP.
#1 is already possible via addons like IgnoreMore, and is accomplished client-side. There is no way for an addon to increase the size of the server-side ignore list.
Just found an addon called ImpIgnore that offers a clientside ignore list with aging. It doesn't offer every feature, like severity levels, integration with blizzard's serverside ignore list, and so on, but is the best compromise I've found.

I've had this idea for a long time, that I'd really like a button on my screen, that when it's on:
- Stops casting of all heal spells on ignored player.
- Stops casting of all buffs and rezzes on ignored player.
Both cases could cause text to flash on screen. The on-screen button would be so I can turn off the function entirely, during guild raids, when you really don't want any extra stuff running.
It just came to mind again last night, after I put 4 guys on ignore for absolutely horrific comments to a female tank in trade chat, graphically describing the abuse she deserved for her gender. Being on a pvp server, I don't want to heal them either out in the world, or in any pug instance or raid. Even better would be if all the healers could share our lists, so we can take over the world.

I was thinking of a priority queue addon for the ignore list:
1. Lets you rank "severety" of the ignore from 1-5
2. If your list is full and you ignore, remove the "least severe" player oldest first.
